I need to add my form to my template

We have a specific recommendation form that our staff have to print and give to their peers to submit back to us.  How do I attach the form so they can download it and print it?

I'm not sure what you meant by "I need to add my form to my template" but if you're asking how to allow someone to print the form, there are 2 ways:

1. Either use the traditional way - Any website/page you visit on the internet can be printed. Most modern browsers have a built-in PRINT function. So, just open the form on your browser, then hit CTRL+P on your keyboard to print it.

2. Or you can add a PRINT BUTTON on the form itself.
