I am past this part.
I am trying to ascertain if my form integration is correct on the URL
provided or is the issue further down the chain. My MailChimp List Is not
populating with the data from the form and it affecting the Zapier that
goes from MailChimp to Pipedrive.

I cloned your latest form and integrated it with my mailchimp account. So far I was able to integrate it successfully and the submission was sent successfully, albeit there was a slight delay. May I ask if you are referring to the submissions in your form before Mailchimp was integrated? Please note that only the latest submissions will be passed on to mailchimp once the form is integrated.
If you have sent new submissions and yet the mailchimp is not receiving the submission data, please let us know. Please give us the link to the specific form where you are encountering this issue.
