How do I format a question into point form and have a mandatory check box for it?

I want to include my return policy on my form. I'd like to format it in point form and have a check box that needs to be checked off before the form is submitted. I'm not seeing how I can format the question in the check box field into a smaller font and into point form. 

I am not sure if I understand your query correctly. Do you mean to have some questions as a checklist and the users need to check them before submitting the form? If so, you may consider adding Checkbox fields to the form enabling the users to check them. You may mark the field as required so that the form is not allowed to submit without filling the required field.
