Why image is not showing in my form?

Hola he creado varios formularios los cuales llevan imagen, pues bien algunos salen las imágenes y en otros no, copio el código exactamente igual y los pego en la misma web.

Alguien me puede ayudar

I checked your form: http://www.jotformpro.com/form/53362618156962 and I see that image is not showing, bu it is showing in your other forms (I checked few of them). 

What I found out is that the URL of the image changed in that form. Could you please try re-uploading the image?

Also, do you have other forms affected by this problem?

Hi,

I have checked your form (53362618156962). I have cloned it to my side.

It is seen that your issue is about the image name. 

Please have a look at the following steps:

#1- I have copied the image source.

 

#2- I have pasted the image source to the address bar but your image not found.

 

As you see that name of your image consists some spaces. Please remove these spaces (and dots) in the image name: Tema 3.126.jpg

Please change the image name and upload your image again.

You can follow the steps below about how to upload image to your form:

1- Basic Fields>Image: Drag and drop the Image form field to your form.

2- Click Upload File in Upload and select your image.

Hola de nuevo, he cambiado el nombre de la imagen varias veces, y el
problema sigue, lo que ocurre que al cargar la imagen aparece el nombre de
la imagen un punto y unos números.
el punto y los números se ponen solo.
Este formulario lleva tiempo colgado en mi web, antes se podía ver la
imagen y hace 2 o 3 días no. Este problema tambien ocurre en otros
formulario, en cambio muchos formularios que tengo se pueden ver la imagen.
Por favor podrían ayudarme?
