Survey form will not update with changes, even if I save it.

Hi, I created a duplicate survey form in order to do some changes, renamed it, made changes to it and am trying to save it but when I preview it, it does not reflect any changes besides the name change. I'd like to avoid creating a new one since I just had to tweak the new one a bit from the old one. If you could help me out, that would be great! 

I checked your form and was able to successfully make updates.  Any changes made were reflected in the form preview.  I cleared your forms cache in case there was an issue with loading a cached version of the form.
