No Forms that are created are working

Good Afternoon, 

All of our forms, when trying to either send as a link or embed in our websites and apps are showing up with a 404 error - form not found. This form is disabled. Can you please help me understand why this is happening and how it can be fixed. This is causing many issues with our clients.

Your account was suspended by one of our form reviewers.  The note that was attached is that one of your forms requested Facebook passwords.  I checked your recent form and did not find any that did so.

I activated your account and your forms are usable once again.  If any of your forms do request usernames or passwords, I would strongly recommend removing those fields to prevent your account from being suspended again.
