I have form not found error

Trying to create new form, but keep getting "form not found" error. 

Kindly let me know how I solve this problem.

Your account status is autosuspended. Your form is asking sensitive information like login credentials or asking for a password. This is a violation of our terms of use.

Actually, if you have a word ("password") on your forms, it will be automatically suspended by our phishing detector tool.

I am re-activating your account but you need to immediately delete the field that is asking for password or you can delete the whole form. Your account might be suspended again if you don't do this immediately.
