 I'd like a form similar to the one at hotwiretoronto.ca/gov but with the option to upload a file, that's why I chose Jotform.

We have a system which detects the phishing fields in Jotform and this system suspends the accounts and disables the forms if there are fields that include phishing. But in rare cases we can encounter with the suspends which are made by mistake. In such cases our users contact us.

I inspected your forms and found that your form has been perceived as phishing which is titled as "Order Form Hotwire Communications" because of the "Driver's Licence Number and Expiry" field. I would like to point out that our users cannot collect sensitive data according to the our terms of use. Because of this reason your account has been suspended automatically by our phishing system.

I removed this field and activated your forms & account.
