Having problems, form is not sending entries to recipient email. Please respond ASAP.

I checked your email history and it looks like one of the recipient emails cburke@ha*******cdc.org was added in the bounce list, which is why you are not receiving the notifications there. I would like to inform you that I have now removed the email address from the bounce list, so the next emails should send successfully to that address.
