First of all thank you for your sensitivity and contacting us. I inspected the form which you have mentioned.

We do not interfere the form content if there aren't any fields that include social security number, credit card info, account passwords etc. So our phishing system has been detected it as phishing but after reviews of one of the team members it has been marked as not phishing. 

However you still think it is a mistake, please inform us again about why do you think this form includes phishing fields.
