why is my account disabled and how can I restore it?

My login is awakemedia (need to retrieve password but there is no account!) Why did I not get an email before disabling?

I checked your forms and they look to be within our terms of use.  It is likely the references to Social media sites that resulted in the suspension.  I have activated your account and your forms are usable once again.  

If any of your forms request username or passwords, I would recommend removing those fields to prevent your account from being suspended once again.
