How do I setup the form that is email to me, to keep the same layout as the form I am emailing? Below is what I receive in an email, when someone fills the form out.

You can change the body of the email notification in the Email settings, below is the step by step instructions on how this can be done once you have open the form for edit in JotForm.
1. Click on "Setup and Embed" tab on the form builder toolbar
2. Click on "Email Alerts"
3. Select Notification
4. In the next window you will see the structure of the notification email as displayed below,
You can edit the body of the email according to your needs.
