But see the output that we are receiving in email. We used one of the templates provided, and edited/customized. The results don't look anything like the field info input on front end.
how do we resolve. Please inform.

I was able to see the issue as well when I checked your form https://www.jotform.us/form/61706746555160 .The current content of the existing Notification E-mail on the form does not match the actual fields in available on the form.
To fix the issue, try first deleting the current Notification email template on the form and create a new one.
the fresh Notifier template should auto update with the existing fields on the form.
Please let us know if this did not resolve the issue.
