    Hello KhanDavid,

Your understanding it correct. Unfortunately it is not possible to have password field in form. Authentication form can't be used in JotForm and creating such forms are against the Terms of Use.

I have deleted the password "passcode" field from your form and activated your account again. Do try to sign in to your JotForm account and get back to us if the issue persists.

Kindly refrain from creating form with email/password, login/password or credit card details.

    Thank you!

    Hello KhanDavid,

Did you try to add those fields again before deleting it?

I did check you form and it seems to be clean. I have activated your account again. Please test your form again and get back to us if the issue persists.
