I have created a form to accept payment for an annual subscription. Then I insert the Stripe payment form element. Then I clicked the integration wizard, and chose Payment Type = Sell Subscriptions. When I click CONTINUE, I see the "Payment Settings" with "Selling Subscriptions" at the top of the page. But then I also see "Please select subscriptions that you would like to add your form." ... but there are no subscriptions listed for me to select. The list box below the "Please select..." text is blank.
How do I create a subscription so that I can select it for use with the form?

The subscription that is being referred to should be found in your Stripe account under Plans.
The image below shows how it should look like if you have saved plan subscriptions in your Stripe account.
If you don't have any saved plan subscription you can click the "Back" button and click "Continue" again to create subscriptions within the Stripe Payment wizard.

Thanks for the detailed walk-through. But I still have no success.
I have created Plans in Stripe...
Then I go back to the JotForm form and click on the Wizard icon and make sure Stripe is connected. (see below)
But I notice that my interface is different that the one you show above. For example, your wizard button is color and mine is gray and white. Does that matter? Am I using the wrong JotForm version?
Then when I click the green [CONTINUE] button at the bottom of the right-hand pane, I see the screen below which lists no subscriptions despite my having plans setup in Stripe. Also I note that the screenshots from your example are different from what I see. Your example shows "PAYMENT WIZARD" in dark text on a white background, with "Selling Subscriptions" underneath. My example shows "Payment Settings" on a dark background with "Selling Subscriptions" underneath.
How can I get access to the same version you are using? Any other tips for how to get the Stripe subscription list to show up in the right pane?

You are using the latest version of JotForm V4 which is in Beta. I will test the issue and get back to you soon.

I'm sorry for the trouble caused to you.
There seems to be some technical glitch where in Jotform V4, Stripe payment tool is not able to fetch subscriptions form integrated stripe account. Please check the screenshot below:
I am reporting it to our backend team. We will get back to you as soon as we have any update from them. For now, I would suggest you to please access older version of UI and then you should be able to list the subscriptions form your stripe account. Please check the screenshot below on how to switch to older version and fetch subscriptions from Stripe:
Please ensure that you enable "Fetch Subscriptions from Stripe Account" option under "Additional Gateway Settings".
