Form not found problem

Hi, 

We have a system which detects the phishing fields in Jotform and this system suspends the accounts and disables the forms if there are fields that include phishing. 

I inspected your forms and found that your forms include some phishing fields. I would like to point out that our users cannot collect sensitive data according to the our terms of use. Because of this reason your account has been suspended automatically by our phishing system.

I removed the password fields and activated your forms & account.
