I have checked your account rejectx23yahoocom and I can see you have not completed the tasks on the "Get More" section yet.
In fact you have not created any form in your account so far, may be you created your form while you were logged into a guest account or while logged into a different account. Could you please share us the link to the form that does not load for you?
We will be glad to check why it does not load.
