Hello again,
As I have understood you want to set an "upload file" field to your form.
Only you have to do is to add an upload file field as you can see the screenshot below:

Actually not upload. I had my test form working but somehow I broke it.
All I want that the user enters their email and hit submit button. The action on submit button should be to send the user an attachment that I have created in PDF.
I had it working but since then I had made several changes and then deleted various conditional logic and in the process I lost what I had setup initially to attach the file on submit.
It's not a live production form so I am not worried much but once I get everything setup correctly, the form will have proper name and it will go in production site.

You can setup an autoresponder email and either send the PDF submission to your customer or your own PDF file. Please see steps below.

Yes. Now I remember. That's what I was looking for.
Next quick question. I don't have to upload PDF somewhere for users to get the PDF. Correct? Also, the PDF will permanently reside here? Because this will be an ongoing submission with attachment.

Hi,
If you set PDF Attachment from "NO" to "YES",with that enabled, your users will receive submission as PDF to their email.
Now, you do not have to upload PDF to Attach File field.
