This makes the second time I have found my email listed above on your blocked list. WHY!!!

This will happen when the delivery of the form email fails to your email address in many cases. To avoid getting this problem, make sure to whitelist Jotform.com domain as a whole and add noreply@jotform.com to your safe sender list.
