I need to know if we are able to create custom forms.
Also, what delivery formats are available? Can we customize delivery for the right format, IE: HTLM, ADF, Specific CRM?
Also, your form is not responsive, why? Are your forms responsive?

Yes you can create custom forms.
This guides can help you to create your forms:
How to create your first web form
You can style your form with CSS:

https://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes
You can also embed your forms to your webpage:

You can also get a full source code of your form and embed it to your webpage. This way you will be able to manipulate with HTML of your form.
Regarding ADF, please check this thread: https://www.jotform.com/answers/180132
We have several CRM integrations: https://apps.jotform.com/category/crm
Also our forms can be responsive. Easiest way to make form responsive is by adding Mobile Responsive widget to your form. You can also check this guide:
How to make mobile friendly forms on JotForm
I would also suggest you to sign up and create free account, before you start creating your forms. You can use this link to sign up: https://www.jotform.com/signup
We usually only respond to one question per thread to avoid confusion. So if you have any followup questions I woulds suggest you to open new thread for each question you have.
Hope this will help.
