I've had a few forms that have had issues with actually sending to the client. One that stands out the most is the "BFS Foster Parent Form". We have had issues with emails not going through properly on some of the conditions and issues with the notification email not being sent out. They contacted me and said they hadn't received a fill-out since December 22nd.
The site we are using these forms on is benchmarkfamilyservices.org and the form is on the contact page as an embedded form.

I examined your email history and found out that several emails are in the bounce list:
I removed these 3 emails from the bounce list, so you may try a submission to test the notifications.
