I've created a form and have made a number of "test" submissions. There are three email addresses set to be notified when a form is submitted. I am not receiving this. I changed my address from firstname.lastname@example.org to email@example.com to see if mindspring(earthlink) was the problem. I have also checked my junk mail folders - no email noting a form submitted.

There seems to be a confusion. The text on the middle is just a name for the email. Since you can have multiple emails. You must change the recipient address from the bottom:
Fixed it on all emails. Please see if that helps.
