Something happened with my card

    I got an email that my card was not up to date but did not see where I could update my card.

    Eric, the subscription was cancelled due to failure to charge the card on file. You can re-upgrade and take advantage of the discounted rates ($114 a year ) with the Bronze plan. Premium plan is no longer offered.

    The only thing I was on some plan that was $45 annually - I am sure that was a grandfathered plan of some kind.

    The submission numbers on the pricing page is that per month or per year?  I have more forms and lower submissions.  I am not sure which plan I would even need.

    Though it was a grandfathered plan, unfortunately, it is no longer available since the subscription is already cancelled. 

    The submission numbers on the pricing page are the limits per month. I see that your account is currently having 61 forms. Since the account is downgraded to Free, you may manage the existing forms without any issue. However, you'll not be able to create any new forms or restore any deleted forms unless you upgrade your account to the plan as per the forms limit. Since your account currently having less than 100 forms, you may consider upgrading your account to Silver subscription plan.
