I am creating an enrollment form and would like the parent to be able to add an additional childs info.

    Unfortunately, we do not have a feature that allows dynamically adding fields on a form. What you can do instead is to create multiple duplicates of such fields, hide them via conditions, and create a checkbox or a radio button that asks the user whether they want to add another field.

    Take a look at this example form I made.

    I used conditional logic to hide the other name fields (Guest 1, Guest 2, ...) and then made them show if the user chooses "Yes" in the "Add another guest" radio button.
