How to create a photography contract?

Im interested to learn how I can create a photography contract and put the embedded info into my zenfolio.com account. 

When the client submits the "submit" button, does it go to my email?

Yes, you will be notified when someone submits to your form. You will receive a copy thru your email of the submission. Here's a guide: Setting-Up-Email-Notifications
