You titled your post as "I need feel a database table on my website with data forms"

I'm assuming you wanted to display a sort of a table on your website that will show your Form's Submission Data.

If that's the case, you can use any of these 2 reports:

    1. How-to-Create-an-HTML-Table-Listing-Report

    2. How-to-Create-a-Grid-Listing-Report

Upon creating a report, you will be given a link to that report. To embed it on your website, just use an iFrame (example).

Now, on the other hand, if you're referring to an actual database, you may send Submission Data to your MySQL Database using PHP. Just refer to your previous thread about this here https://www.jotform.com/answers/902811
