I don't remember my user name or my password.

I don't have access to the email account that the jot form is linked to. I also do not remember my user name or password.

How would i go about resetting that ?

I checked your website getsojo.com and found a form there that was created with jotform. From this I was able to identify that the form embedded on your website is owned by an account with the username sojo. The email address related to that account is info@getsojo.com.

You can change your password by clicking the forgot password link from the login page and entering either your username or the email address. The password reset instructions will be sent to info@getsojo.com.
