I have two recently created forms that tested fine, and were used by one of our rescue partners on 12/30/2016. Now both are no longer functioning, as we can't get beyond the first page.

I cloned your form and I was able to replicate the issue with the conditions on the radio button not showing the page breaks. This may be due to HTML tags that are added on the radio button options. The tags are visible in the condition, but they are not visible in the actual form. Because they don't appear similar, the conditions are not being triggered correctly.
To resolve the issue, please remove the html tags on the radio buttons.
Once the tags are removed, the conditions should work correctly now.

I removed the code to make the text red, and everything is OK now. I've used bold in the radio buttons for quite some time, but only recently added red text.
