Remove third option

I've created a contact form with radio button question with two options (that allow users to state a preference between phone and email contact) but it continues to show a third option: "Option 3." This seems like a bug. How can I have removed permanently?

Also, is there a way to configure the redirect message or redirect page after users submit the form?

Finally, the form is supposed to be encrypted but does not appear to be so as I still get email notices. Can you verify for me?

Under the radio button Properties, you need to remove the unwanted option under "Options" tab and save the changes as shown in the video below  :
