Form is Disabled

    My forms are showing up as disabled. As far as I read on a thread, this could have gotten caught in a phishing filter of yours.

    I am collecting passwords from a client of mine in which I need.

    Please get this corrected as soon as possible.

    Your account has been auto suspended because it has been tackled by our phishing algorithm. When I check your forms I can see that you are asking for username and password data on them. This is forbidden on JotForm due to security measures. Please check our Terms of Use to avoid further issues related with phishing since repeated violation of them may lead to a permanent suspension of your account. 

    I have now deleted the related fields on your form and reactivated your account.
