When I click on the form, nothing happens, a blank screen shows.

It seems that you are a sub-account user of another account. Could you provide us with the form ID or the web page URL where the form is embedded so that we can take a look? As I check your recent edited form "RTC Intake Call Sheet", I see that the form is displayed normally at my end. If you can provide us with more information on the issue that you are experiencing we can assist you further.
