I checked your form and it is working fine. I did a test submission and it was sent successfully. The submit button is working properly. Here's a screenshot:

You need to add the email address (email@example.com) to the notification email alert. Right now Tlfarrior@gmail.com is receiving emails after a submission. Here how you can add your email to the notification alert:
If you want you can delete the other email address from the notification alert.
