Why are my forms not working?

Hello, i want to know why my forms aren't working. Did i have to delete my accont and make another one?? If i do need to delete.. i want to, please.

This could be because of your form http://www.jotformz.com/form/70031774953659 which is asking for login information.

But I see that you have recently deleted the form. I have activated your account. The forms should be accessible again now.
