i am getting form is disable after submited form field.Form not foundThis form is disabled.

I have whitelisted your forms to avoid this from happening again. I would suggest you to translate your form to English also, because the phishing system is not updated to scan forms made in other languages: https://www.jotform.com/help/298-How-to-Make-your-Forms-Multilingual
