Can a form be sent to different people based on how it is filled out?

I offer personal training and massage and I want to create an online form for this.  I offer these services at multiple locations. 

Can the form be emailed directly to the location people are requesting the service if we have only one form for Personal Training and one for Massage.  Could they select the location in the form and then it was sent to that location.
