I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that your form titled "Fast Secure Reload" is having a field to collect a 16 digit card number which is being suspected. Please be noted that it is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

Please provide us with more information on the card number that you are collecting so that we'll see what best we can do in this regard. We are sorry for the inconvenience this may have caused.

We will wait for your response. Thank you!
