I had set up an integration that stopped working for some reason, so I removed the integration and went to set it up again, but it keeps failing. Any ideas?

I am able to reproduce this on my end and have alerted our developers to the issue. We will let you know via this thread as soon as it has been resolved.

The integration should now be working once again. If you have any further trouble integrating your form with Google Docs, let us know and we will be happy have another look.
