I need a table with check boxes and text

    Is is possible to have a table that has questions, check boxes and text boxes all in one?

    Hello,

    There are various checkbox options. I prepared a form including different checkbox options. Feel free to clone it and inspect it:

    You can add a widget to your form:

    1) Click Widgets tab

    2) Search for "checkbox";

    Hi Seth and thanks for the quick reply. Actually what I am trying to do is this...

    Question 1                CheckBox         Comments

    Question 2                CheckBox         Comments

    etc...

    Where Question is static text, Checkbox is Required and Comments are Optional. I have tried to add to matrix side by side but am struggling with them looking like one matrix.

    Hello,

    Unfortunately it is not possible to have a matrix field with checkboxes and input fields at the same time.

    You can create these fields one by one and style according to your needs. You will need to inject CSS code to have a solid looking for your fields. This will take some time and effort.

    If you can effort to not to add the comment box, you can use the widgets that I suggested. Otherwise, I think, there is no ready to use widget or element you can use.

    You can add a comment box at the end where users can leave a comment on the form.

    Hi, I added the comments at the end as you suggested ass this looks better than side-by-side matrix.
