Currently when a form is submitted the corresponding questions and answers are provided in the body of the email. But the PDF attachment only has the answers submitted. Is their a way to get the same info in the body of the email to show up in the attachment?

You can customize the PDF attachment to your email notification by going to PDF > Customize PDF in the submissions page of the form in questins. Please see below.
If you need further assistance, let us know and just tell us which one of your form needs fixing and we will be glad to help.
- gllandryAnswered on January 06, 2017 at 05:45 PMThanks for the response but I am still having trouble. I wanted to add the questions (see attached) to the left hand column on the PDF. I am not able to edit these fields.

It seems you have attached a sample screenshot of the issue you are encountering but we weren't able to see the screenshot. Can you directly attach/upload the screenshot on this thread so we can properly see it? You can access the thread here: https://www.jotform.com/answers/1026942. To add a screenshot on this thread, just follow the instructions provided in this article: How to add screenshots images to questions to the support forum.
We will wait for your response.
