I am not sure if I have understood your question correctly. Do you mean to say that you get "invalid refresh token" error message after the form is submitted?
I could not test your form as it requires active credit card. I have cleared your form cache form here. I would suggest you to please re-integrate your form with infusionsoft and get back to us if the issue persists.
