Upon checking your account I found that it's suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.
Upon taking a closer look at your form, I can confirm that it's not being used for any phishing activities. I have activated your account now. Do try to sign in to your JotForm account and get back to us if you have any trouble.

Hi, thanks for reactivating. Can you please tell me my your system regarded me as phishing so that I can avoid this happening again in the future?

To avoid your account from being suspended in the future, please make sure to avoid fields that don't have any labels and forms should have proper titles. Also avoid adding password and credit card fields in your form.
