When trying to download form submissions as PDF, the photo images are failing to download. Instead of the photo, the PDF has a small empty box where the photo should be. See attachment.

I have cloned your form and tested at my end. As I tried to download the PDF report of the test submission, I am able to see that the image is displaying normally at my end.
As I check your JotForm submissions, I see that the images are not displayed in the submissions view as well. Did you make any changes to your form or the submissions or changed the account username recently? When I tried to send a test submission to your form, I see that image is displaying properly.
Could you try submitting a test submission to your form from your end and see if that works well?

Still not working on my end. Your submission downloaded to PDF no problem. I made a test submission and it failed.
Also, the images are still displaying in the submissions view on my end.

I was able to reproduce the issue you're having when I made a test submission with your form. And I believe the issue only occurs if the photo is uploaded to your form from a phone. It seems to, however, work if the photo is uploaded from your form on a computer.
You will notice if the photo is uploaded from a phone or a device in your submission page.
Although I was able to reproduce the issue when I tested your form, I'm not able to reproduce it on a test (newly created) form. So I tried to update (open the widgets wizard and click the update button) the widget on your form, but that doesn't seem to fix it. Can you also try to create a test form on your account, add the "Take Photo" widget, and make test submissions using your phone to test if this happens to any forms your create on your account?
I will forward this concern to our developers so they would be able to check it further. Although we cannot provide any ETA to when this will be fixed, I can assure you that if we find anything we will update you here, in this thread, the soonest.

I made a new form called "Take Photo Test Form" and created a test submission. The photo fails to download to PDF from this new test form also. The test submission was made using an Android tablet.

I am able to replicate the issue with the new form. I see that the issue is already been forwarded to our backend team. Once the issue is fixed, you'll be posted here with any update we have.
