How to change my form's page title?

    How do I modify the tab title.  I have used an old form as a template, and when it opens in a new tab (or a preview if I copy and paste the link) it shows the original title (2015...).  I'd like the tab to read the name of the current form.  Or I can manually alter it, if I could only find where to do that.
