Can't submit form

I have 3 forms that are live (I think), and none of them work.  I filled them out and pressed submit button.  The button quickly changes and I can see "please wait" for a split second and then nothing happens.  No submissions are reported.  Am I missing a step?

never mind, 10 min later... seemed like it took care of itself. 

Thanks for informing us that it's working now. However, seeing that this form is really short, it should only take a couple of seconds to submit it and not 10 minutes.

So, I still ended up cloning your form to test it real quick and it submitted in just a few seconds. Here's how it went from my perspective:
