Our form is not working in Safari. We have tried a number of computers with the same result. What is the issue?Page URL:

I could not replicate the issue at my end. I checked the page on Safari as well as Chrom and it just loaded fine for me. here is the screenshot:
I would suggest you to clear the form cache and the browser cache and check again.
