How to create this kind of forms?

Are you referring to the form displayed on the web page?

It is possible to create such form as displayed here by using our Form Designer. Some of the themes already available in our theme store may also be helpful for you. However, the original form on the web page is connected a database and searching for data after clicking now. Unfortunately, searching data is not possible with JotForm. However, you may consider using the source code of the form so that you may connect to the database for any custom operations.
