Calculation is not displaying decimal values

Also, one of my sub-total calculations (under 2nd Participant) is not placing the decimal and two spaces after the number (ie: 12.00)

I entered it the same way as the other two but it's not working properly...can you take a look and suggest what to do to fix it?

I have checked your JotForm and see that the settings on the Calculation widget seems to be fine. However, it seems that the issue with the field. When I tried to add a new calculation widget, it seems to be working fine on the cloned version of your JotForm. Please try removing the existing Calcuation widget and add a new widget to see if that works for you.
