How can I make my survey look correct?

    I had my rep at Zenfolio insert the code for a contact form that I created in JotForm. It looks pretty wonky and they sent me back to you to correct it.

    I had checked the form on the link provided by you. To me, it appears to be an integration issue.

    Please fetch the code again as described at  https://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use and try embedding it again from scratch.

    If you still face the similar issue again, then I would recommend you to use IFRAME code
