How do I get a check box to show if it is checked on my submission page?

    At the bottom of the form we have added a check box for permission to use quotes.  When the submission is sent through I cannot see if the box is checked or not.  Is there another type of field I should be using instead of a check box?  Here is the form:

    Unfortunately, it is not possible to display Checkbox in the submission page. You may consider using a Radio button with options like Agree/Don't Agree so that the corresponding value will be displayed in the submission. 

    You may also consider using a condition to hide the Submit button unless the checkbox is checked. Please refer to the guide below that can help you with setting up a condition to show/hide fields based on user's response.
