I have set up your forms for use with our charity. Then the password was erased and also the user name forgotten and so far your retrievel system has refused to function. As we cannot have faith in your system we hsall not be using your software for our projects. NONE of your links work and it locks us out of the recovery sequence. Coupled with NO tele support i think we shall give your software a wide berth in future.

I've searched your account using the email address above and as far as i can see you are using JotForm as a guest. I was able to locate 4 different forms. That is the reason why you can't reset your password. You just need to create a free account using the same email address above and your forms will be automatically transferred to your registered account.
